How We Serve

We use a formal process of gathering information about our clients, including but not limited to, individual characteristics, presenting problems, history or cause of illness, history of treatment, bio-psycho-social history, and current functioning in emotional, cognitive, social, and behavioral domains through face-to-face, contact with the client and/or family. This service results in the identification of the client’s mental health or addictive disease service needs and recommendations for treatment which may include a tentative diagnosis.

Community-Based Behavioral Healthcare

RTS Services Unlimited, Inc. is a full-service counseling facility. We serve children, adolescents, and adults throughout the Will County and surrounding areas. RTS offers one-on-one support and training in homes, schools and the community. RTS’s direct-care staff supports clients with specialized counseling, educational supports, and skill-building to help restore their spirits, increase problem-solving abilities, enhance social skills and define coping mechanisms. RTS uses an individualized and a family- centered approach.

Terms and Conditions of RTS Services Unlimited, Inc.
1. Introduction
These Terms and Conditions (“Agreement”) govern the provision of services by RTS Services Unlimited, Inc. (“RTS,” “Company,” “We,” “Us”) to clients, contractors, and other stakeholders. By engaging with RTS, using our services, or communicating with us via SMS messaging, you agree to abide by these terms.
2. Service Provision
• RTS provides mental health and behavioral health services in accordance with state and federal regulations.
• Services are available to all individuals regardless of race, religion, gender, ethnicity, disability, or sexual preference.
• Clients shall receive services based on their individual needs, cultural background, and accessibility requirements.
3. Client Rights and Responsibilities
• Clients shall be treated with dignity, respect, and fairness at all times.
• Clients will receive a Client Bill of Rights upon admission, outlining their entitlements, grievance procedures, and privacy rights.
• Clients have the right to participate in their treatment planning and to withdraw from services at any time with written notice.
• Clients must provide accurate and truthful information regarding their mental health history and agree to comply with service recommendations.
4. Confidentiality and Privacy
• RTS strictly adheres to HIPAA regulations and other federal and state confidentiality laws.
• Client information will not be disclosed without written consent, except as required by law.
• RTS ensures secure handling, storage, and transmission of client health records and protected health information (PHI).
• SMS Messaging and Privacy:
o By opting in to SMS messaging, clients agree to receive service-related notifications, appointment reminders, and other RTS communications.
o RTS does not share phone numbers or SMS correspondence with third parties.
o Clients may opt out of SMS messaging at any time by replying “STOP” to any RTS message.
o Standard messaging rates from the client’s mobile provider may apply. RTS is not responsible for message delivery failures due to carrier restrictions.
5. Fees and Payment Terms
• Fees for services are determined based on Medicaid eligibility, private insurance coverage, or self-pay options.
• Clients will be informed of all fees prior to service initiation.
• Fees for document reproduction may apply when records are requested by third parties.
6. Contractor and Staff Obligations
• RTS contracts with independent contractors, who must adhere to the same professional standards as employees.
• Contractors must sign a Business Associate Agreement and Confidentiality Agreement before providing services.
• Contracts are reviewed annually, and non-compliance with RTS standards may result in termination.
7. Termination of Services
• Either party may terminate services with 10 business days’ written notice.
• RTS reserves the right to suspend or terminate services if a client engages in policy violations, fraudulent activities, or behaviors that endanger staff or other clients.
• Contractors failing to meet compliance standards may have their agreements revoked.
• Clients who opt out of SMS notifications will still be responsible for attending scheduled appointments unless they provide prior notice.
8. Grievances and Dispute Resolution
• Clients and contractors may file complaints using the Grievance/Complaint Form provided at intake.
• RTS will investigate complaints within 48 hours and provide a formal response.
• If unresolved, grievances may be escalated to the Illinois Office of Inspector General.
9. SMS Messaging Consent and Usage
• By providing a mobile number, clients agree to receive SMS messages related to their treatment, including appointment reminders, service updates, and emergency alerts.
• RTS will not use SMS messaging for marketing or promotional purposes.
• Clients may unsubscribe from SMS messaging by replying “STOP” at any time.
• RTS is not responsible for SMS messages that are delayed, lost, or misdelivered due to carrier issues.
• RTS does not guarantee SMS confidentiality; clients should avoid sharing sensitive information via text.
10. Amendments and Policy Updates
• RTS reserves the right to modify these Terms and Conditions at any time.
• Clients and contractors will be notified of any changes in writing or via SMS.
11. Liability and Indemnification
• RTS is not responsible for safeguarding client valuables.
• RTS does not transport clients in personal vehicles.
• By engaging in RTS services, clients and contractors agree to indemnify RTS against any claims arising from non-compliance with these terms.

Effective Date: January 1, 2025

1. Purpose

RTS Services Unlimited, Inc. (“RTS”) is committed to protecting the privacy and security of personal information, including information collected through SMS (text messaging) communications. This policy outlines how RTS collects, uses, stores, and protects SMS-related data.

2. Consent to Receive SMS Messages

By providing your phone number to RTS and opting in to SMS communication, you consent to receive text messages regarding:

  • Appointment reminders and scheduling updates
  • Service-related notifications
  • Important announcements and policy updates
  • Crisis support and resources
  • Billing and payment reminders (if applicable)

3. Information Collected

When you opt-in to receive SMS messages from RTS, we may collect:

  • Your mobile phone number
  • Message content (responses, inquiries, confirmations, and any additional information you provide)
  • Timestamps of messages sent and received

4. How We Use SMS Data

RTS uses SMS messages solely for the following purposes:

  • Facilitating client communication related to treatment and care
  • Confirming or rescheduling appointments
  • Providing program updates and critical information
  • Notifying clients of service availability or changes
  • Responding to client inquiries

RTS will not send promotional or marketing messages via SMS unless you have explicitly opted in to receive them.

5. SMS Data Security

RTS employs reasonable administrative, technical, and physical security measures to protect SMS data from unauthorized access, loss, misuse, or disclosure. However, RTS cannot guarantee that SMS communications are completely secure due to potential risks inherent in mobile networks.

Client Responsibilities:

  • Avoid sharing sensitive personal information via SMS (e.g., Social Security numbers, medical diagnoses, or financial details).
  • Report any unauthorized messages claiming to be from RTS immediately.

6. Third-Party Disclosure

RTS does not sell, trade, or rent SMS-related data to third parties. However, we may share SMS data with:

  • Service providers that facilitate SMS communication on our behalf, under strict confidentiality agreements
  • Law enforcement or legal entities if required by law or to protect the safety of our clients and staff

7. Opting Out of SMS Communications

Clients can opt out of SMS messages at any time by replying STOP to any message. Once opted out, RTS will no longer send SMS messages unless you re-enroll.

8. Message Frequency & Costs

  • Message frequency varies based on client needs and engagement with RTS services.
  • RTS does not charge for SMS messages, but standard carrier message and data rates may apply. Clients are responsible for any fees charged by their mobile carrier.

9. Changes to This Policy

RTS reserves the right to update this SMS Privacy Policy at any time. Clients will be notified of any significant changes through our website or via SMS. Continued use of RTS SMS services after such updates constitutes acceptance of the revised policy.
